To obtain permanent subscriber manager license/s please complete all sections and submit the completed form. If you have any problems please contact your EION Wireless Account Manager.

In Section 1, enter the details of the person and company who is requesting the license.

In section 2, enter the information required about the network to be licensed.

In Section 3, enter the MAC address of the Primary and Standby (if applicable)Server requiring a license.
